Thank you for scheduling your follow-up Functional Medicine appointment with Dr. David Schwartz.  You will soon receive a confirmation email and/or text message from us with the date and time you reserved.

Here are a few reminders to prepare for your appointment:

➕ Please arrive at least 15 minutes early to your follow up appointment.  There may be a progress form to fill out upon arrival.

➕ If you’d like this appointment to be VIRTUAL, notify us by emailing staff@triadhealthcenter.com or call 336-288-4677.

➕ If you have had any lab testing, imaging, or treatment plans outside of Dr. Schwartz’s recommendations since your last visit, please bring copies to this appointment.

➕ You may like to bring a notebook, pen and water bottle.

➕ Wear comfortable clothing and shoes that you can easily stand and sit in.

➕ If you have any questions since your last appointment, please email them in advance to us: staff@triadhealthcenter.com
