Thank you for scheduling your appointment.  You should already have received a confirmation email and/or text message from us with the date and time you reserved.

Here are a few reminders to prepare for your appointment:

➕ Download the INTAKE PAPERWORK packet.  Have this filled out completely and bring with you to your appointment.

➕ Bring your Insurance Card(s) and photo ID with you.

➕ If this is a pediatric appointment (ages 11 and younger), download the PEDIATRIC PAPERWORK. Have this filled out completely and bring with you to your child’s appointment.

➕ If this is a pediatric appointment, at least one guardian must attend.

Plan to arrive at least 15 minutes before your scheduled visit time. Upon arrival, check in with our Front Desk team. Our medical assistant will collect your INTAKE PAPERWORK forms and scan your insurance card(s) and photo ID. Please have these cards available for her.

If you have any questions about your paperwork or consultation we will be glad to answer them when you arrive.

After a brief tour of our office, you will meet with our doctors in the consultation room.  You will first have a consultation so we can understand your health history and current health goals. Our doctors will then complete thorough diagnostic exams and testing as needed, including digital X-rays if recommended. We recommend you dress comfortably for your exam, as the doctors will need easy, direct access to areas of complaints, as well as surrounding structures.

Following your intake appointment, a follow up will be scheduled to review the doctors’ findings, treatment plan, and insurance coverage, at no additional cost.  We will show you our plan to get you back on the path to optimal health!
